You may qualify if you...

  • 18 years of age or older
  • At least 2 years post-kidney transplant
Not Eligible

You will not qualify if you...

  • Not a kidney transplant recipient (e.g., liver, lung, heart, intestine, pancreas, bone marrow transplant)
  • Unable to measure own blood pressure and glucose if diabetic
  • Unable to use a mobile health application after training
  • Unable to speak, hear, or read English
