Who Can Participate
Eligibility Criteria
You may qualify if you...
- Self-report as African American/Black
- Age 18 years or older
- Screen positive for one or more adversities using the Health-Related Social Needs Screening tool
- Self-reported type 2 diabetes and self-report or screen positive for chronic kidney disease
- Able to communicate in English
You will not qualify if you...
- Cognitive impairment at screening visit
- Active psychosis
- Active alcohol or drug abuse/dependency
