Eligibility Criteria

Eligible

You may qualify if you...

  • Self-report as African American/Black
  • Age 18 years or older
  • Screen positive for one or more adversities using the Health-Related Social Needs Screening tool
  • Self-reported type 2 diabetes and self-report or screen positive for chronic kidney disease
  • Able to communicate in English
Not Eligible

You will not qualify if you...

  • Cognitive impairment at screening visit
  • Active psychosis
  • Active alcohol or drug abuse/dependency
