Researchers are evaluating the effectiveness of the Ultrasound-guided Modified Thoracoabdominal Nerve Plane Block (M-TAPA) for managing postoperative pain in patients undergoing Laparoscopic Sleeve Gastrectomy (LSG), a common bariatric surgery. The study focuses on whether M-TAPA can reduce opioid use within the first 24 hours after surgery, which is important since opioids can cause complications like breathing problems, nausea, and delayed recovery. This research is led by Istanbul Medipol University Hospital and aims to improve pain management and recovery in this patient group. The study compares two groups: one receiving the M-TAPA block along with standard postoperative pain care, and the other receiving only the standard pain management without the nerve block. The M-TAPA procedure involves ultrasound guidance to inject local anesthetic near the ribs to block specific nerve branches. Pain medications including ibuprofen, tramadol, and fentanyl via a patient-controlled system are used in both groups. The M-TAPA block is performed after surgery while the patient is still under general anesthesia. Participants will be monitored for opioid consumption and pain levels at multiple time points within 24 hours after surgery. Rescue pain medication use and any adverse events during this period will also be recorded. The study uses randomized, double-blind methods to compare outcomes. The total participation includes the surgery, treatment, and close monitoring for the first day after the operation to assess pain relief and medication needs.

Duration - Day of surgery and 24 hours after surgery
Participants undergo laparoscopic sleeve gastrectomy under general anesthesia. For those in the experimental group, the M-TAPA block is performed after wound closure while still under anesthesia. All participants receive standard postoperative pain management, including medications and a patient-controlled analgesia system.
