Study on Improving Psoriatic Arthritis Screening Using Modified PEST Questionnaire in Adults with Moderate-to-Severe Plaque Psoriasis in Canada

This research aims to evaluate the effect of adding two questions and pictures to the validated Psoriasis Epidemiology Screening Tool PEST for diagnosing psoriatic arthritis PsA in adults with moderate-to-severe plaque psoriasis PsO in Canada. The study focuses on patients who are candidates for biologic Disease-Modifying Antirheumatic Drugs bDMARDs. It seeks to improve screening methods for PsA in this population. Participants will complete a modified PEST questionnaire called PESTpictures2 at their first dermatologist visit. This tool includes the original PEST questions, two extra questions about morning stiffness and night pain, and pictures illustrating joint symptoms. Those who screen positive will have a follow-up visit with a rheumatologist to confirm a PsA diagnosis. An end-of-study remote visit with the dermatologist will document participants bDMARD treatment choices and status within up to 120 days. During the study, participants will fill out screening questionnaires and attend up to two clinical visitsone with a dermatologist and one with a rheumatologist if neededfollowed by a remote end-of-study visit. Researchers will measure how well the modified PEST predicts PsA diagnosis, patient experience with the questionnaire, and treatment decisions. The study will also collect surveys and interviews from dermatologists to assess the screening tools impact and acceptability.
