This research will compare the effectiveness of a remotely delivered personalized exercise coaching plus an evidence-based sleep improvement intervention to remotely delivered personalized exercise coaching alone for knee osteoarthritis pain. The study team hypothesize that the combined intervention will result in greater improvements in patient-reported pain intensity, recorded with real-time data capture, than remotely delivered exercise coaching alone.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Self-reported physician diagnosis of knee osteoarthritis
- Persistent knee osteoarthritis-related pain of at least moderate intensity
- Persistent symptoms of insomnia as determined by Sleep Condition Indicator questionnaire
- Stable pain or sleep medication prescription for at least three months prior to study and willingness to maintain medication during study
- Adequate literacy to complete web-based surveys independently
You will not qualify if you...
- Diagnosis of systemic, inflammatory musculoskeletal disorders (e.g., rheumatoid arthritis)
- Active cancer
- Neurological conditions such as movement disorders
- Medical conditions preventing graded increase in physical activity as advised by a healthcare professional
- Insufficient English comprehension to participate
- Unable to provide electronic informed consent
- Significant cognitive impairment (2 or more errors on 6-item cognitive screener)
- Severe psychiatric disorders
- Participation in another interventional trial or pain/sleep management program within past six months
- Visual or hearing impairments preventing use of intervention
- Serious physical health issues requiring surgery or life expectancy under 6 months
- Irregular sleep-wake schedules such as shift work
- Untreated specific sleep disorders other than insomnia (e.g., sleep apnea, narcolepsy)
- Meeting or exceeding physical activity guidelines from U.S. Department of Health and Human Services
