Nephrotic Syndrome (NS), including Minimal Change Disease (MCD), Focal Segmental Glomerulosclerosis (FSGS), and Membranous Nephropathy (MN), causes a significant financial and health burden, contributing to about 12% of end-stage renal disease (ESRD) cases. This study aims to better understand these rare diseases by collecting long-term observational data and biological samples to improve knowledge and care strategies. Some pediatric participants under 19 years old with limited prior immunosuppressive treatment and not scheduled for biopsy can also join. Study involvement includes consenting for an additional kidney biopsy core during clinically indicated procedures for tissue analysis. During the study, researchers will monitor changes in urinary protein levels and kidney function over 60 months. Various health outcomes such as quality of life, infections, hospitalizations, kidney injury, and other medical events will also be tracked. Participants are followed regularly with data and specimen collection to support research into better understanding and managing these kidney diseases.

Eligibility Criteria

Eligible

You may qualify if you...

  • Patients with an early clinical diagnosis of FSGS, MCD, or Membranous Nephropathy
  • Documented urinary protein excretion of 1500 mg or more per 24 hours, or equivalent protein:creatinine ratio
  • Scheduled for a clinically indicated kidney biopsy (for biopsy cohort)
  • Children under 19 years old with less than 30 days of immunosuppression therapy
  • Proteinuria or nephrotic syndrome confirmed by urine analysis and serum albumin criteria (for pediatric non-biopsy cohort)
Not Eligible

You will not qualify if you...

  • Prior solid organ transplant
  • Diagnosis of glomerulopathy without diagnostic kidney biopsy
  • Clinical, serological, or histological evidence of systemic lupus erythematosus (SLE)
  • Presence of other kidney diseases such as Alport syndrome, Nail Patella syndrome, diabetic nephropathy, IgA nephritis, monoclonal gammopathy, or genitourinary malformations
  • Known systemic disease with life expectancy under 6 months
  • Inability or unwillingness to provide informed consent
  • Inability or unwillingness to follow study procedures and visit schedule
  • Institutionalized individuals, such as prisoners

Diagnostic Evaluation

Duration - Day of biopsy

Participants scheduled for a clinically indicated kidney biopsy undergo the biopsy procedure, which includes collection of an additional renal core for research purposes.

Long-term Monitoring

Duration - Up to 60 months

Participants are followed with regular assessments to monitor urinary protein excretion, renal function, and other health outcomes over time.

Visits occur periodically over 60 months
