Regional citrate anticoagulation will be used throughout the blood circuit. The control group will receive standard CKRT therapy alone as appropriate. Treatments will be monitored over time to assess safety and efficacy. Throughout the study, patients will be closely monitored using clinical assessments and laboratory tests. The primary outcome is a combined measure of mortality or dialysis dependency at 90 days after treatment. Secondary outcomes include dialysis dependence at one year, ICU-free days within the first 28 days, and mortality at 28 days. Patient safety will be observed continuously, and the total study participation may extend over several months to evaluate long-term effects of the therapies.

Eligibility Criteria

Eligible

You may qualify if you...

  • Admitted to an ICU requiring continuous kidney replacement therapy (CKRT)
  • Acute Kidney Injury stage 2 or greater at the time of CKRT initiation
  • On CKRT for at least 12 hours but no more than 48 hours at enrollment
  • Age between 18 and 80 years at enrollment
  • Presence of at least one additional life-threatening organ dysfunction
  • Acceptable vascular access suitable for CKRT, including adequate catheter size and length
  • Initial non-binding commitment to maintain current level of care for at least 96 hours
  • C-Reactive Protein greater than 3.5 mg/dl

You will not qualify if you...

  • Not expected to survive the next 24 hours
  • Anticipated transition to comfort measures or hospice within 4 days
  • Terminal condition with expected survival less than 28 days or considered futile by the investigator
  • Active treatment of advanced malignancy or palliative chemotherapy or radiation
  • ICU hospitalization longer than 14 days during current admission
  • Active COVID-19 infection as primary admission diagnosis
  • Chronic use of ventricular assist devices
  • End-stage renal disease requiring chronic kidney replacement therapy
  • History of chronic kidney disease greater than Stage 3
  • Acute Kidney Injury stage 0 or 1 at CKRT initiation
  • Non-acute tubular necrosis diagnosis of AKI
  • Acute coronary syndromes, stroke, or major vascular events needing intervention within 48 hours of randomization
  • Active bleeding requiring blood transfusions at screening
  • Acute on chronic liver failure
  • Suspicion of hepato-renal syndrome
  • History of any solid organ transplant prior to admission
  • Severe burns with modified Baux score over 100
  • Bone marrow transplant within the last year
  • Chronic immunosuppression with prednisone over 20 mg/day or equivalent for 30 days prior to admission
  • History of primary or secondary immune disorders including HIV or AIDS
  • Dry weight over 150 kg
  • Platelet count less than 15,000/mm3
  • Prisoner or member of a vulnerable population
  • Pregnant or breastfeeding
  • Concurrent enrollment in another interventional clinical trial
  • Need for plasmapheresis

Participants receive continuous kidney replacement therapy (CKRT) with or without the Selective Cytopheretic Device (SCD) for acute kidney injury. Those in the experimental arm will have up to ten sequential 24-hour treatments with the SCD in-line with their CKRT circuit.

Participants are monitored for outcomes including mortality, dialysis dependency, and recovery over several months after treatment.
