Researchers will evaluate frailty using the FRAIL Scale as the primary outcome and measure functional status, depressive symptoms, number of falls, emergency visits, and hospitalizations as secondary outcomes. The programs delivery will be monitored for consistency, and participants health progress will be closely tracked throughout the study period lasting at least 18 weeks.
