What this Trial Is About

The trial investigates a novel adapted multilevel intervention called D-THIO (Deprescribing Thyroid Hormone In Older Adults) aiming to support reducing or stopping thyroid hormone use in adults aged 65 and older who may be overtreated or misusing thyroid hormone. This study will evaluate how well D-THIO works compared to enhanced usual care in helping providers deprescribe thyroid hormone and reduce overtreatment, while also exploring knowledge and attitudes about deprescribing among providers and patients across multiple sites. D-THIO includes evidence-based pharmaceutical opinion letters sent to providers and educational brochures given to patients, facilitated by clinical champions. Providers are randomized to either receive D-THIO or enhanced usual care, which consists of American Thyroid Association guidelines and brochures. Patients of these providers receive corresponding educational materials. The trial enrolls 150 providers and 750 patients across three sites, with randomization at the provider level. Participants will be monitored through surveys of providers and patients assessing knowledge, attitudes, and discussions about deprescribing up to 12 months after enrollment. Researchers will measure thyroid hormone deprescribing by providers and reductions in overtreatment via medical records 6 months post-enrollment. The study includes interviews with stakeholders to understand implementation factors. Overall participation lasts from enrollment through 6 to 12 months of follow-up, with safety and efficacy assessed through clinical data and surveys.
