What this Trial Is About

This research investigates the outcomes of a suspension uretheroileal technique during open radical cystectomy and ileal neobladder reconstruction. The study compares this technique with the conventional method to evaluate differences in continence rates at 1, 3, 6, and 12 months after the procedure. The condition being studied is radical cystectomy, often performed for muscle-invasive or certain high-grade non-muscle-invasive bladder carcinomas in patients aged 18 to 80 years. The suspension uretheroileal technique involves ligating the complex of the dorsal vein complex and puboprostatic ligaments, sharply dividing it from the prostate, and then reconstructing the ileal pouch by everting the mucosa and suturing it outward. The neobladder neck is narrowed to about 1 cm to allow passage of a 20 F catheter. Anastomotic sutures are placed at specific clock positions through the full thickness of the urethra. The key difference in the suspension technique is that two sutures are anchored to the ligated complex to suspend the poucho-urethral anastomosis, unlike the conventional approach. Participants will be monitored over 12 months to assess continence outcomes at multiple time points following surgery. The study includes detailed surgical procedures, and researchers will evaluate the success of the suspension technique compared to the conventional method. The total participation involves follow-up visits to measure urinary continence and assess recovery progress, ensuring careful observation of patient outcomes after the procedure.
