Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adults with high-impact chronic pain as defined by the Graded Chronic Pain Scale - Revised
- Currently prescribed long-term opioid therapy for more than 90 continuous days
- Have at least one opioid safety concern including any of the following:
- Moderate-to-high dose opioid prescription (morphine equivalent daily dose above 60mg)
- Comorbid conditions increasing opioid risk such as chronic pulmonary disease, sleep apnea, chronic kidney disease, chronic hepatitis, or cirrhosis
- Active high-risk co-prescriptions like benzodiazepines, gabapentin (1800mg or higher), or pregabalin (150mg or higher)
- Substance use risk or potential opioid misuse indicated by urine toxicology or substance use disorder diagnoses
- Positive TAPS or AUDIT-C screening scores
- Presence of adverse events from opioids
- Score above 8 on the Prescribed Opioids Difficulties Scale (PODS)
You will not qualify if you...
- Moderate to severe cognitive impairment as measured by the Blessed Orientation Memory Concentration screening tool
- Current or recent (past 6 months) prescription or use of buprenorphine
- Inability to read or understand English
- Severe hearing or speech impairments preventing participation in phone interviews or coaching
- Terminal illness with life expectancy less than 12 months
- Planned move outside the treatment areas of the study sites
- Currently working with or worked with a Whole Health Coach in the past 6 months
- Major surgery planned during the study or follow-up period
- Recent suicide attempt within 90 days before screening
