The objective of this study is to adapt the Patient Priorities Care (PPC) framework to breast cancer survivorship via a user-centered approach, through an iterative process in which patients and their physicians help to refine and modify the intervention. A second objective is to evaluate the feasibility and effectiveness of the adapted PPC framework in breast cancer survivorship for older adults.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Able to write and speak English
- Diagnosed with stage DCIS, I, II, or III breast cancer
- Completed active cancer therapy 3 months to 10 years ago
- Have at least 3 documented chronic conditions or take 10 or more medications, or have had at least 1 hospitalization, or at least 2 emergency visits, or seen more than 2 specialists in the past year
- Able to provide consent
You will not qualify if you...
- Currently receiving active cancer therapy, including chemotherapy, radiation, surgery, or multimodal treatment
