Eligibility Criteria
You may qualify if you...
- Child is between 2 and 18 years old
- Has at least 1 complex chronic condition (CCC)
- Takes 5 or more current medications, including prescription, as-needed, and over-the-counter
- Receives primary care at Children's Hospital Colorado
You will not qualify if you...
- Non-English speaking
- Sibling or family member already enrolled in this study
