Who Can Participate
Eligibility Criteria
You may qualify if you...
- On the Penn Medicine Primary Care Service Line registry
- Last 2 outpatient systolic blood pressure readings 60 140 within the past 12 months
- Diagnosis of ASCVD or ASCVD risk score 60 10% or diagnosis of diabetes or A1c 60 6.5% in the last year or on diabetes registry or last LDL 60 190 within past five years
- Not currently taking PCSK9 inhibitors or inclisiran
- Not taking statins or on low- or moderate-intensity statins with LDL 60 100
You will not qualify if you...
- Currently taking PCSK9 inhibitors
- Documented allergy or intolerance to statins in medical record
- Pregnant or breastfeeding
- Life expectancy markedly shortened by metastatic cancer, hospice care, end stage renal disease, congestive heart failure, or dementia
- Non-English speaker requiring a translator
- Do not have a cell phone
