Eligibility Criteria

Eligible

You may qualify if you...

  • On the Penn Medicine Primary Care Service Line registry
  • Last 2 outpatient systolic blood pressure readings 60 140 within the past 12 months
  • Diagnosis of ASCVD or ASCVD risk score 60 10% or diagnosis of diabetes or A1c 60 6.5% in the last year or on diabetes registry or last LDL 60 190 within past five years
  • Not currently taking PCSK9 inhibitors or inclisiran
  • Not taking statins or on low- or moderate-intensity statins with LDL 60 100
Not Eligible

You will not qualify if you...

  • Currently taking PCSK9 inhibitors
  • Documented allergy or intolerance to statins in medical record
  • Pregnant or breastfeeding
  • Life expectancy markedly shortened by metastatic cancer, hospice care, end stage renal disease, congestive heart failure, or dementia
  • Non-English speaker requiring a translator
  • Do not have a cell phone
