Pilot Study Comparing Circulating Tumor DNA in Cancer Draining Vein Versus Peripheral Vein Samples During Biopsies for Hepatobiliary and Pancreatic Cancers

Researchers are investigating whether detecting circulating tumor DNA ctDNA can be improved by collecting blood samples from the vein draining the cancer area compared to the usual method of sampling from a peripheral vein. The study focuses on patients undergoing biopsies for hepatobiliary and pancreatic cancers, including hepatocellular carcinoma, cholangiocarcinoma, ampullary cancer, and pancreatic carcinoma. This is a prospective pilot study conducted at a single center aiming to enhance ctDNA detection accuracy. Participants with suspected primary hepatobiliary or pancreatic cancer who are scheduled for a diagnostic percutaneous or trans-jugular biopsy will have blood collected from both the cancer draining vein and a peripheral vein during the biopsy procedure. This additional blood collection involves obtaining 10 mL of blood from the draining vein. The study compares ctDNA results from these two blood sources and also compares them with standard biopsy results using next generation sequencing. During the study, researchers will measure the amount of ctDNA in both draining vein and peripheral vein plasma samples by comparing the ratio of mean allele frequency over one year. They will also analyze the number of unique genetic alterations found in the different blood samples and compare these to biopsy-derived sequencing results. Participants will undergo their scheduled biopsy and blood sampling, and the study includes monitoring and evaluation of ctDNA for up to one year after sampling.
