The goal of this observational study is to identify factors in blood that are associated with response to deep brain stimulation (DBS) surgery in patients with Parkinson's disease. The main questions it aims to answer are: 1. What factors in blood (proteins and RNA) are associated with good vs poor response to DBS? 2. Are these factors able to predict response to DBS? 3. How do these factors change before and after DBS? Blood and leftover brain tissue (which spontaneously adhere to the surgical instruments) will be taken and routine clinical data (including scores from routine assessments) will be collected from consenting participants who undergo DBS.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Diagnosis of clinically established Parkinson's disease as defined by Movement Disorder Society criteria
- Referred for deep brain stimulation based on local clinical guidelines
- Significant motor complications despite optimized medication
- UPDRS motor score greater than 30 out of 108 in the off-medication state
- Hoehn and Yahr stage greater than 2.5 out of 5 in the off-medication state
- Responsive to dopamine with more than 33% improvement in UPDRS motor score after levodopa
- Age 75 years or younger
- No contraindications to surgery or serious health conditions limiting life expectancy
- No significant psychiatric problems or cognitive impairment
- No structural brain lesions or signs of atypical Parkinsonism on imaging
You will not qualify if you...
- Unwilling to provide blood samples for the study
- Parkinsonism caused by heavy metal exposure
- History of neurodevelopmental or other neurodegenerative diseases besides Parkinson's
- Central nervous system infections or inflammatory diseases such as multiple sclerosis or CNS lupus
- Malignancy within the last 10 years
- History of stroke, HIV infection, or systemic autoimmune diseases
- Alcohol dependence or other substance abuse
- Unable to pass pre-operative assessment for deep brain stimulation or unwilling to undergo the surgery
