Study on Why Clot-Busting Treatment May Not Work in Adults with Acute ST-Elevation Heart Attack

Researchers are investigating why thrombolytic treatment using tissue-plasminogen activator t-PA sometimes fails in patients with acute ST-segment elevation myocardial infarction STEMI. This observational study aims to better understand factors linked to unsuccessful thrombolysis, which is an important issue especially in regions where many patients receive this treatment before hospital arrival instead of immediate invasive procedures. Understanding these factors can help improve risk assessment for patients undergoing thrombolysis. The study will include 200 STEMI patients who receive prehospital thrombolysis and arrive at Oslo University Hospital Ullevl. Upon arrival, a blood sample will be collected to analyze biomarkers related to blood clotting, clot breakdown, and inflammation. These biomarker levels will be compared between patients who experience successful thrombolysis and those for whom the treatment fails. Participants will be involved from hospital admission, with immediate blood sample collection to measure relevant biomarkers. Researchers will assess whether thrombolysis was successful or not based on clinical criteria decided right after hospital admission. The study will monitor these outcomes to better understand why some treatments fail. The study is expected to enroll patients between 2022 and 2024, with follow-up measures focusing on thrombolysis success.
