Eligibility Criteria

Eligible

You may qualify if you...

  • Hospitalized patients aged 65 or older
  • Prescribed at least seven medications at admission recorded in the Electronic Patient Journal (EPJ)
Not Eligible

You will not qualify if you...

  • Terminal patients or those with a short life expectancy
  • Patients residing outside the study region
  • Patients discharged on the day of inclusion
  • Patients who do not speak Danish
