What this Trial Is About
Researchers are evaluating two surgical reconstruction methods for patients with proximal gastric adenocarcinoma, a type of stomach cancer. The study compares proximal gastrectomy with anterior anastomosis plus pyloroplasty to esophagogastric anastomosis, focusing on their effects on gastroesophageal reflux disease, postoperative quality of life, and both short- and long-term outcomes. This interventional trial aims to provide insight into which surgical approach may better manage these patients' conditions. Participants undergo proximal gastrectomy surgery with either anterior anastomosis combined with pyloroplasty or an alternative esophagogastric anastomosis where the connection is placed at the posterior or side of the gastric stump. The anterior anastomosis approach includes pyloroplasty as part of the procedure. Participants are randomly assigned to one of these two surgical groups, and the trial is conducted under single masking. During the study, researchers assess gastroesophageal reflux disease using pH monitoring and questionnaires six months after surgery. Surgical complications are monitored for one year, and long-term outcomes are followed for up to five years. Participants' organ function and performance status are evaluated for eligibility, ensuring appropriate monitoring of safety and recovery throughout the trial period, which started in 2021 and runs through 2027.
