What this Trial Is About

Psychosis is a major cause of disability worldwide, affecting millions of people. This research focuses on individuals with First Episode Psychosis (FEP), a critical early phase of psychosis that often involves repeated relapses and is linked to risks of self-harm and suicidal thoughts. The trial aims to evaluate the feasibility and acceptability of culturally adapted psychological therapies designed to address suicidal ideation in this group, particularly in settings like Pakistan where mental health services face challenges such as lack of trained staff and resources. The study assesses a combined intervention called CMAP Plus CBT, which integrates Culturally Adapted Manual Assisted Problem Solving (CMAP) with Culturally adapted Cognitive Behavioral Therapy (CaCBT). This approach involves 12 therapy sessions over three months, including evaluation of self-harm attempts, psycho-education, crisis skills, problem-solving, and thought management. The intervention is based on established CBT principles and tailored to cultural contexts. Participants will be monitored from the start of the study through 12 weeks of intervention. Researchers will track feasibility and acceptability indicators, assessing how practical and well-received the therapy is for individuals with FEP experiencing suicidal thoughts. The study includes assessments using standardized scales and close monitoring of symptoms and safety throughout the intervention period.
