Eligibility Criteria
You may qualify if you...
- Signed and dated informed consent form provided online
- Male or female aged 18 years or older
- Affected by pulmonary fibrosis or interstitial lung disease as one of the following: patient (including post lung transplant), caregiver (current or past), or family member (parent, full or half-sibling, or child)
- Has internet access and a valid email address
You will not qualify if you...
- Primary residence or place of care is outside of the United States
- Unable or unwilling to provide informed consent or comply with the study protocol
- Any condition or circumstance that may add risk, interfere with participation, or affect data quality as judged by the investigator
- Diagnosis of sarcoid, lymphangioleiomyomatosis (LAM), pulmonary alveolar proteinosis (PAP), cystic fibrosis (CF), or amyloidosis (for patients, caregivers, and family members)
