Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 5 to 18 years
- Diagnosis of type 1 diabetes for more than 12 months
- Suboptimal glycemic control at enrollment, with hemoglobin A1c above 8% or average blood glucose over 200 mg/dl in the past month
- Plans to receive diabetes care at the UCD Pediatric Diabetes Clinic for the next year
- Access to internet with a device that has video and audio capabilities
- Ability to connect home glucose meter, continuous glucose monitor, or insulin pump to an internet device via Bluetooth or cable
You will not qualify if you...
- Home address outside of California due to physician licensing restrictions for telehealth services
