The goal is to find non-invasive markers that can help diagnose kidney problems without the need for biopsy in the future. This research includes three groups: one with patients undergoing kidney biopsy where tissue is stored and followed every 3 months for the first year, then yearly; a second group of patients who had biopsies without stored tissue or have diabetic kidney disease without biopsy, followed annually; and a third group of kidney transplant patients needing biopsy for monitoring or clinical reasons, assessed at biopsy time. Blood, urine, and serum samples are collected at visits for research purposes. Control subjects without kidney disease can also be included with data and specimen storage. Participants will provide informed consent and undergo regular evaluations including collection of blood, urine, and serum samples. Researchers will monitor changes in disease progression for up to 10 years. The data collected will include demographic, clinical, histological, genetic, and imaging information. This long-term observational study allows comparisons across different kidney disease states and control subjects to discover new predictive markers and disease mechanisms.

Eligibility Criteria
You may qualify if you...
- Male or female adults aged 18 years or older
- Willing and able to provide informed consent
- Require an initial kidney biopsy for medical necessity (Native Biopsy Tissue Group)
- Previously had a kidney biopsy without tissue stored in this repository or have diabetes with kidney disease and no biopsy (Native Kidney, Non-tissue Group)
- Have undergone a kidney transplant and require a transplant biopsy for surveillance or clinical reasons (Allograft Tissue Group)
