What this Trial Is About
Endovascular therapy (EVT) added on best medical management is currently recommended in acute large vascular occlusion (LVO) stroke patients with National Institutes of Health Stroke Scale (NIHSS) score \>5. Thus, a sizeable fraction of patients with a minor stroke that do not undergo cerebrovascular screening may experience an early neurological deterioration (END) due to LVO, possibly leading to poor long-term functional outcome. However, whether these patients may still benefit from a rescue EVT is unknown, especially in a late window (\>24 hours). In this study, the investigators assume that best medical management plus EVT might be superior than best medical management alone in a late window for minor stroke patients who have experienced an LVO and END. The primary objective of the study was to establish the safety and efficacy of EVT in a late window for minor stroke patients in the anterior circulation who experienced an LVO and END.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 years or older
- Presenting with symptoms of acute ischemic stroke and initial NIHSS score less than 6
- Symptoms worsened within 7 days of first stroke onset
- Randomization occurs more than 24 hours after stroke onset
- Time from symptom worsening to randomization is 24 hours or less
- NIHSS score before randomization is 6 or greater
- Signed informed consent
- Imaging shows blockage in the internal carotid artery terminus or middle cerebral artery M1 segment
- Symptom progression due to recurring blood flow problems in the same brain region
- CT scan score (ASPECTS) before randomization is 6 or greater
- Perfusion imaging shows low blood flow in target vessel area with core infarction volume under 50ml, mismatch rate at least 1.8, and mismatch volume over 15ml
You will not qualify if you...
- Pre-stroke modified Rankin Scale score greater than 1
- Symptom worsening caused by brain hemorrhage, edema, or other clear causes seen on imaging
- Target vessel too small, tortuous, or difficult to treat with endovascular methods
- Severe narrowing or blockage of multiple blood vessels
- Untreated brain aneurysms, tumors (except small meningiomas), or vascular malformations
- Brain hemorrhage within past 6 months
- Recent bleeding in gastrointestinal or urinary system, heart attack, brain injury, or major surgery within past month
- Known bleeding disorders or abnormal blood clotting
- Severe heart, liver, or kidney disease or other serious late-stage illnesses
- Severe allergies to contrast media not controlled by medication
- High blood pressure not controlled by treatment
- Uncontrolled blood sugar levels
- Pregnant women or women of childbearing potential with positive pregnancy test
- Expected survival less than 1 year due to serious illness
- Participation in other conflicting clinical trials
- Any condition that makes study participation unsafe or not feasible, including mental or cognitive disorders
