Risk Factors for Extubation Failure in Children on Mechanical Ventilation in Pediatric Intensive Care at Sohag University Hospitals

Mechanical ventilation MV is commonly used in about 40% of pediatric intensive care unit PICU patients to support breathing. This research investigates the risk factors for extubation failure EF, which occurs when patients need to be reintubated within 48 hours after removing the breathing tube. EF is linked to serious complications including pneumonia, lung damage, longer hospital stays, and increased risk of death. The study aims to better understand these risks to improve care for critically ill children. The study observes PICU patients under 18 years old who have been on mechanical ventilation through an endotracheal tube for more than 12 hours and are ready to wean off the ventilator based on specific clinical criteria. Patients will be grouped into those who successfully pass spontaneous breathing trials and extubate without needing reintubation within 48 hours, and those who fail and require reintubation. Researchers will monitor these groups to identify factors linked to extubation success or failure. Participants will be closely evaluated during their time in the PICU with monitoring of breathing, oxygen levels, heart rate, and other vital signs. The main outcomes measured are whether extubation and weaning from mechanical ventilation are successful or fail within 48 hours. Secondary outcomes include survival over 30 days. This observational study collects detailed clinical data to help develop better guidelines for weaning children from mechanical ventilation and reducing complications.
