What this Trial Is About
Patient: Tear film issues resulting in poor visual outcomes are a major cause of dissatisfaction after cataract surgery. There is no universal best preoperative algorithm for managing the ocular surface prior to surgery. Industry: Recent advances in tear/cornea imaging and refractive cataract surgery such as multifocal intraocular lenses will be advantageous to patients only if tear film issues can be addressed. Society: Cataract surgery is the most common ophthalmic surgery done in Singapore and worldwide. Improving vision by attending to tear film problems improves quality of life.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age between 21 and 99 years
- For dry eye patients, must meet at least one: symptomatic dry eye with DEQ5 >6 and either tear break-up time less than 5 seconds or staining greater than grade 1 in any corneal zone
- Use of diquafosol, cyclosporine, or tacrolimus eyedrops for dry eye within the last 2 weeks and previously started for more than 2 months, OR current visible lower punctal plugs in at least one eye
- For control patients without dry eye, must meet all: DEQ5 score less than or equal to 6
- Staining less than grade 1 in all corneal zones
- No use of diquafosol, cyclosporine, or tacrolimus eyedrops in the last 2 months
- No visible punctal plugs
You will not qualify if you...
- Presence of another ocular condition affecting vision as much as cataract, such as active corneal stromal disease, scar, or significant corneal endothelial disease
- Known active thyroid dysfunction, Sjogren disease, or rheumatoid arthritis
- Intraocular surgery within the previous 6 months
- Laser-assisted in situ keratomileusis or pterygium surgery within the previous 3 months
- Use of central nervous system or hormonal drugs within the last month and during the study
- Active ocular infection, inflamed chalazion, or presence of pterygium
- Glaucoma requiring topical anti-glaucoma medications
- Need to wear contact lenses during the study
- Conditions that make follow-up difficult (e.g., wheelchair use)
- Pregnant or breastfeeding women
