This randomized split-mouth clinical trial will compare the effectiveness of 38% silver diamine fluoride (SDF) versus 5% sodium fluoride varnish (FV) in reducing salivary Streptococcus mutans levels in children with bilateral caries. Each participant will receive SDF on one side of the mouth and FV on the contralateral side. The primary outcome is change in Dentocult SM Strip Mutans scores in plaque over 3 months. Secondary outcomes include plaque index, bleeding on probing, and adverse events.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 3 to 8 years; cooperative behavior (Frankl score 2 or higher)
- At least two contralateral primary molars with active dentin caries (ICDAS 3-5) of similar severity
- Parent or guardian consent and child assent when appropriate
You will not qualify if you...
- Allergy to silver
- Presence of ulcerative gingivitis or stomatitis
- Use of systemic antibiotics within the past 4 weeks
- Fixed dental appliances that interfere with plaque assessment
- Any condition that the clinician judges would prevent safe participation
