This study evaluates a sensor-controlled digital game (SCDG) to motivate self-management behaviors of weight monitoring and physical activity in adults with heart failure (HF). Half of the participants will receive the SCDG app and weight monitoring and physical activity sensors and the other half will receive only the weight monitoring and physical activity sensors.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 45 years or older
- New York Heart Association (NYHA) heart failure classification I to III allowing minimal physical activity without discomfort
- Experienced heart failure hospitalization in the past 12 months
- Fluent in English
- Passed a mini-cognitive screening test
- Score less than 2 on the Outcome and Assessment Information Set item for ambulation/locomotion (able to walk independently without cane or walker)
You will not qualify if you...
- Severe visual impairments (e.g., legally blind) or severe tactile impairments (e.g., severe arthritis) preventing use of smartphone or sensors
- History of renal failure affecting heart failure prognosis
- Diagnosis of end-stage or terminal illness (e.g., cancer)
- Previous heart transplantation or implantation of durable mechanical circulatory support device (e.g., left ventricular assist device) due to unique self-management needs
