Eligibility Criteria
You may qualify if you...
- Documentation in electronic health record of any type of sickle cell disease
- Owns a mobile device compatible with the intervention
- Has access to the internet
You will not qualify if you...
- Plans to relocate outside of the study site area within the next 12 months
- Plans to transition to adult care within 12 months or less
- Parent, caregiver, or provider reports neurocognitive impairment that prevents use of the intervention or participation
- Non-English speaking
