Who Can Participate
Eligibility Criteria
You may qualify if you...
- Aged 60 or above
- Recently hospitalized with a stay of 2 days or more
- At risk of functional decline within 3 months after hospitalization as measured by SHERPA score above 3.5
- Discharged home without referral for exercise-based rehabilitation
- Owns a smartphone capable of video calls
- Has given consent to participate
You will not qualify if you...
- Hospitalized with disabling conditions causing significant function loss such as stroke
- Bed-bound or chair-bound
- Has conditions that prevent exercise training (e.g., acute musculoskeletal problems, unstable cardio-respiratory diseases)
- Engaged in moderate or vigorous exercise more than 60 minutes per week in the past 6 months
