This is a Phase 3, 38-week, randomized, double-blind, placebo-controlled, multicenter, outpatient study in subjects with psychosis associated with Alzheimer's Disease. Who Can Participate
Eligibility Criteria
You may qualify if you...
- Aged 55 to 90 years at screening
- Able to understand the study and provide informed consent, or have a legally acceptable representative provide consent
- Meet clinical criteria for possible or probable Alzheimer's Disease
- Have a brain MRI or CT scan within the past 5 years to rule out other central nervous system diseases, or complete one during screening
- Living at the same location for at least 4 weeks before screening and intend to stay there during the study
- Able to move independently or with assistance and have an identified caregiver or study partner with frequent contact (at least 10 hours per week) who can attend visits and help with compliance
- History of psychotic symptoms meeting International Psychogeriatric Association criteria for at least 2 months before screening
- Clinical Global Impressions-Severity scale score of 4 or higher at screening and baseline
- Meet criteria for moderate to severe delusions or hallucinations based on Neuropsychiatric Inventory-Clinician scores at screening and baseline
- Mini-Mental State Examination score between 6 and 24 at screening
- If taking cholinesterase inhibitors or memantine, stable dose for at least 6 weeks before screening and willing to maintain during study
- Willing and able to attend outpatient clinic visits and follow study instructions
- Body mass index between 16 and 40 kg/m2
- Female participants must not be pregnant or breastfeeding, and women of childbearing potential and their male partners must use effective contraception during and for at least 30 days after the study
You will not qualify if you...
- Psychotic symptoms primarily due to conditions other than Alzheimer's Disease
- History of major depressive episode with psychotic features within 12 months before screening
- History of bipolar disorder, schizophrenia, or schizoaffective disorder
- Significant or severe medical conditions that could affect safety or study participation, including pulmonary, hepatic, renal, hematologic, gastrointestinal, endocrine, immunologic, dermatologic, neurologic, cardiovascular, or cancer-related diseases
- Severe kidney impairment with estimated glomerular filtration rate below 50 mL/min
- History of ischemic stroke within 12 months or any hemorrhagic stroke
- History of cerebral amyloid angiopathy, epilepsy, CNS tumors, unstable thyroid function, or unexplained fainting
- Certain heart conditions including congestive heart failure class 2, angina grade 2 or higher, sustained ventricular arrhythmias, implantable defibrillator, or recent myocardial infarction
- Personal or family history of long QT syndrome
- HIV infection, liver diseases or active liver infections
- History or high risk of urinary retention, gastric retention, or narrow-angle glaucoma
- History of bladder stones or recurrent urinary tract infections
- For male participants: high prostate-specific antigen levels or severe urinary symptoms
- History of obstructive gastrointestinal disorders or serious constipation requiring treatment in past 6 months
- Risk of suicidal behavior during the study
- Significant abnormal findings on physical exam, ECG, or lab tests
- Positive urine test for substances (except limited cannabis or benzodiazepines use with approval)
- Use of certain psychoactive medications unless stable or able to wash out
- Unsuitable for enrollment due to safety or compliance concerns
- Positive COVID-19 test within 2 weeks before or at screening
- Unable to discontinue medications that prevent study participation
- Prior exposure to KarXT or adverse reactions to trospium
- Participation in other clinical studies involving experimental drugs within 3 months or more than 2 studies in past year
