The main aim of this study is to find out how well elritercept works in lowering the need for RBC transfusions. Other aims are to learn how well elritercept works in reducing the need for RBC transfusions over longer periods of time or in adults with high transfusion needs. A Study of Elritercept to Treat Anemia in Adults With Very Low, Low, or Intermediate Risk Myelodysplastic Syndromes (MDS) Who Need Regular Blood Transfusions
Who Can Participate
Eligibility Criteria
You may qualify if you...
- Ability to understand the study and provide informed consent
- Male or female aged 18 years or older at consent
- Diagnosis of very low, low, or intermediate risk MDS confirmed by bone marrow aspirate
- Dependence on red blood cell transfusions in the 16 weeks before randomization, defined as either 4 to 7 units or 8 or more units within that period
- Transfusion events only counted if pretransfusion hemoglobin is less than 10 g/dL
- At least one transfusion in each 8-week block and minimum two transfusions separated by 7 days within 16 weeks before randomization
- No consecutive 56-day period without RBC transfusion in the 16 weeks before randomization
- Refractory or intolerant to prior ESA treatment or unlikely to respond based on serum EPO levels
- Less than 5% blasts in bone marrow aspirate at screening
- Eastern Cooperative Oncology Group performance status of 0 to 2
- Agree to use effective contraception if of childbearing potential or sexually active male
- Able and willing to comply with study requirements
You will not qualify if you...
- Diagnosis of del(5q) MDS or therapy-related (secondary) MDS
- Anemia caused by other known reasons such as thalassemia, hemolytic anemia, bleeding, or vitamin deficiencies
- RBC transfusions in the 16 weeks before randomization for reasons other than MDS
- Significant cardiovascular disease including advanced heart failure, prolonged QT interval, uncontrolled hypertension, recent arrhythmia, myocardial infarction, or unstable angina
- Ejection fraction less than 35%
- Child-Pugh class C liver impairment
- Recent stroke, deep vein thrombosis, or pulmonary embolism within 6 months
- History of acute myeloid leukemia
- Prior malignancies other than MDS unless disease-free for 5 years, with certain skin and cervical/breast cancers allowed
- History of solid organ or bone marrow transplant
- Active infection requiring recent intravenous or oral treatment
- Known chronic HIV, hepatitis B, or hepatitis C infection
- Body mass index of 40 or higher
- Major surgery within 28 days before randomization
- Allergy or anaphylaxis to study drug components or recombinant proteins
- Prior use of elritercept, luspatercept, sotatercept, or certain MDS treatments
- Recent start of iron chelation, vitamin B12, folate, androgen, or high-dose corticosteroid therapy
- Treatment with other investigational drugs recently
- Participation in another interventional clinical study
- Serum EPO level over 500 U/L
- Abnormal platelet counts or low neutrophil count
- Elevated liver enzymes or bilirubin (except Gilbert's syndrome)
- Low ferritin, folate, or vitamin B12 levels
- Severe kidney impairment
- Pregnant or breastfeeding
- Any other condition that could interfere with study participation or data interpretation
- Study staff and immediate family involvement
- For France: persons under court protection, not affiliated with social security, or legally protected adults
