Who Can Participate
Eligibility Criteria
You may qualify if you...
- Locally advanced or metastatic and/or unresectable hepatocellular carcinoma confirmed by biopsy or clinical criteria
- Disease not suitable for curative surgery or local therapies
- No prior systemic treatment for advanced or unresectable hepatocellular carcinoma
- At least one measurable untreated tumor lesion
- ECOG performance status of 0 to 2 within 7 days before treatment
- Child-Pugh B7 or B8 cirrhosis at screening and within 7 days before treatment
- Adequate blood counts and organ function
- Life expectancy of at least 12 weeks
- Women of childbearing potential must agree to avoid pregnancy and egg donation
- Absolute neutrophil count ≥1.0 x 10^9/L without growth factor support
- Platelet count ≥50,000/µL without transfusion
- Hemoglobin ≥80 g/L (8 g/dL)
- AST and ALT ≤5 times upper limit of normal
- Serum bilirubin ≤3 times upper limit of normal
- Creatinine clearance ≥50 mL/min by Cockcroft-Gault formula
- Serum albumin ≥20 g/L (2.0 g/dL) without transfusion in past 3 months
- INR ≤2.3
You will not qualify if you...
- Pregnancy or breastfeeding
- Prior treatment with CD137 agonists or immune checkpoint inhibitors
- Investigational therapy within 28 days before study treatment
- Locoregional liver therapy within 28 days before study treatment or unresolved side effects
- Treatment with systemic immunostimulatory or immunosuppressive medications
- Live vaccine within 4 weeks before study treatment
- Uncontrolled high blood pressure
- Active or past autoimmune disease or immune deficiency
- History of pulmonary fibrosis, organizing pneumonia, drug-induced or idiopathic pneumonitis, or active pneumonitis on chest CT
- Concurrent progressing malignancy requiring treatment or recent malignancy history that excludes participation
- Known fibrolamellar or sarcomatoid hepatocellular carcinoma, or mixed cholangiocarcinoma and hepatocellular carcinoma
- Symptomatic or untreated central nervous system metastases
- Prior allogeneic stem cell or organ transplant
- Currently listed for liver transplantation
- Co-infection with hepatitis B and C viruses
- Untreated or high-risk esophageal or gastric varices
- Bleeding events from varices or grade ≥3 hemorrhage within 6 months before treatment
- Active hepatic encephalopathy symptoms or instability within 3 months
- History or planned transjugular intrahepatic portosystemic shunt (TIPS) placement (excluded in Cohort A)
- Multiple paracenteses restrictions as specified
- Recent spontaneous bacterial peritonitis within 12 months
