What this Trial Is About
This study is designed to assess the efficacy and safety of ifinatamab deruxtecan (I-DXd) in patients with unresectable advanced or metastatic esophageal squamous cell carcinoma (ESCC) who have experienced disease progression following treatment with a platinum-based systemic therapy and an immune checkpoint inhibitor (ICI) compared with investigator's choice of chemotherapy (ICC).

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Participants aged 18 years or older
- Histologically or cytologically confirmed unresectable locally advanced or metastatic esophageal squamous cell carcinoma
- Disease progression after platinum-based chemotherapy and immune checkpoint inhibitor treatment, with no more than one prior systemic therapy for advanced disease
- Provide adequate baseline tumor samples with sufficient quality and quantity
- At least one measurable lesion on CT or MRI according to RECIST v1.1, not from previously irradiated sites unless progression is documented
- Eastern Cooperative Oncology Group performance status of 0 or 1 within 7 days prior to treatment start
You will not qualify if you...
- Prior treatment with orlotamab, enoblituzumab, other B7-H3 targeted agents, or ifinatamab deruxtecan
- Prior treatment with any topoisomerase inhibitor
- Histologically or cytologically confirmed adenosquamous carcinoma subtype
- Ineligibility for all comparator chemotherapies due to progression or intolerance
- Tumor invasion into adjacent organs with increased bleeding or fistula risk
- Active brain metastases, spinal cord compression, or leptomeningeal carcinomatosis requiring treatment
- Cerebrovascular accident, transient ischemic attack, arterial thromboembolism, or pulmonary embolism within past 6 months
- Clinically significant corneal disease
- History or current interstitial lung disease or pneumonitis requiring corticosteroids
- Severe pulmonary compromise from illnesses or autoimmune disorders
- Chronic steroid treatment of 10 mg daily or more prednisone equivalent, except low-dose inhaled, topical, or intra-articular steroids
