Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adults admitted to the hospital with decompensated heart failure eligible for early discharge with community paramedic support and monitoring
- Acutely decompensated heart failure patients at home or in ambulatory clinic requiring large volume diuresis without need for hospital-level monitoring
- Stage D advanced heart failure patients needing frequent (more than weekly) diuretic adjustments as inpatient or outpatient
You will not qualify if you...
- Hospital-level monitoring or care is clinically indicated
- Failed safety assessment, active substance abuse, or behavioral health diagnosis impacting participation
- Enrolled in hospice care
- Resident in a skilled nursing facility
- No primary care physician or cardiologist at Mayo Clinic
- Communication barriers due to medical illness or cognitive impairment
- Pregnant
