Participants will be involved in regular symptom monitoring through automated calls or texts and follow-up assessments. Researchers will measure time to first hospital readmission within 90 days as the primary outcome, along with secondary outcomes including readmission frequency, days alive and out of hospital, corticosteroid and antibiotic use, healthcare contacts, quality of life, and cost-effectiveness. Safety monitoring will include adverse events and antimicrobial resistance over an average of 4 years, with participant involvement lasting up to one year for some assessments.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 40 years or older
- Recently discharged from hospital after admission for a COPD exacerbation lasting 4 hours or more
- Able to provide written informed consent
You will not qualify if you...
- Required invasive ventilation during hospital admission
- Expected survival less than 90 days
- Signs of new consolidation on chest X-ray (if available)
- Discharged to a residential or nursing home
- Unable to manage a supported self-management plan
- No access to telephone
- Currently participating in another interventional trial
- Previous participation in the RAPID trial

Participants receive either a supported rescue pack with education and twice-weekly automated telephone symptom reminder calls for 90 days, or no rescue pack with standard care. This aims to help manage COPD exacerbations after hospital discharge.
