Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adult patients aged 50 to 80 years with advanced heart failure (NYHA Class III or IV) diagnosed by a physician
- Family caregivers aged 45 to 80 years
- Alert and able to consent to participate
- Able to read and understand English
You will not qualify if you...
- Currently received or waiting for a heart transplant or left ventricular assist device (LVAD)
- Diagnosed with a terminal illness or dementia such as Alzheimer's disease
