What this Trial Is About
The purpose of this research study is to evaluate whether telerehabilitation targeting arm movement, when added to usual care, improves arm function and reduces global disability after stroke, compared to usual care alone. Patients with arm weakness due to stroke that happened in the past 90-150 days will be randomized into one of two groups: \[1\] TR and usual care; \[2\] usual care only (no TR), but people in the usual care group will be offered TR once the study is done. TR consists of 70 minutes/day of activities targeting arm function, 6 days a week for 6 weeks.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 to 80 years at the time of randomization
- Radiologically confirmed ischemic or intracerebral hemorrhage stroke with onset 120±30 days before randomization
- Upper arm weakness after stroke with Action Research Arm Test score between 18 and 44 at baseline
- Box & Block Test score of at least 1 block in 60 seconds with affected arm at baseline
- Able to perform all three example rehabilitation exercises at baseline
- Signed informed consent and behavioral contract by the participant
You will not qualify if you...
- Major active neurological, psychiatric, or medical disease reducing ability to comply with study procedures
- Unable or unwilling to perform study procedures or therapy or expected noncompliance
- Diagnosis other than index stroke substantially affecting the affected arm function
- Severe depression with Geriatric Depression Scale score greater than 10 at baseline
- Significant cognitive impairment with Montreal Cognitive Assessment score below 22 unless due to aphasia and allowed by site PI
- Communication deficits interfering with study participation
- Severe upper extremity spasticity with contracture or modified Ashworth Scale score of 4 in biceps or pectoralis
- Modified Rankin Scale score greater than 2 before the index stroke
- New symptomatic stroke since index stroke or separate stroke within 30 days before index stroke
- Visual acuity worse than 20/50 in both eyes
- Life expectancy less than 9 months
- Pregnant or women of childbearing potential without negative pregnancy test
- Botulinum toxin to affected arm within prior 3 months or expected during study
- Concurrent enrollment in another therapy-based investigational study overlapping with this study
- Insufficient English or Spanish to comply with study procedures and telerehabilitation instructions
- Expected inability to remain at a single residence within 1.5-hour drive of study site during therapy period unless waived
- Contraindication to MRI
- On isolation precautions such as active COVID-19 infection
