The investigators will enroll 1000 patients with chronic LBP seeking outpatient care at the healthcare systems in Maryland (Johns Hopkins Medicine (JHM)) and Utah (University of Utah (UU) and Intermountain Healthcare (IHC)). Eligible patients will provide informed consent and be randomized to receive telerehabilitation or in-clinic physical therapy delivered by a trained physical therapist. Primary effectiveness outcome is the difference in change in LBP-related disability (Oswestry Disability Index) after 8 weeks of treatment.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Primary care visit in the past 90 days with a low back pain-related diagnosis
- Moderate or higher pain and disability with Oswestry score of 24% or more and average pain rating of 4 or higher out of 10
- Chronic low back pain lasting more than 3 months and occurring at least half the days in the past 6 months
- Ability to speak and understand English or Spanish (Spanish only at Utah sites)
You will not qualify if you...
- Lumbar spine surgery within the last 6 months
- Possible non-musculoskeletal cause for low back pain symptoms such as pregnancy
- Serious conditions causing low back pain like cancer, inflammatory diseases, or vertebral infections
- Neurological disorders causing severe movement problems or psychotic disorders such as schizophrenia
- Known pregnancy
