What this Trial Is About
This phase II trial evaluates the safety and effectiveness of giving immunotherapy (nivolumab and ipilimumab) before surgery for controlling disease in patients with stage I-IIIa sarcomatoid mesothelioma. Immunotherapy with monoclonal antibodies, such as nivolumab and ipilimumab, may help the body's immune system attack the cancer, and may interfere with the ability of tumor cells to grow and spread. Giving immunotherapy before surgery may be more effective at controlling disease in patients with sarcomatoid mesothelioma than giving immunotherapy alone.

Testing the Addition of Immunotherapy Before Surgery for Patients With Sarcomatoid Mesothelioma
Who Can Participate
Eligibility Criteria
You may qualify if you...
- Diagnosed with sarcomatoid or sarcomatoid-dominant (over 50%) biphasic pleural mesothelioma
- Stage I to IIIA disease as defined by standard cancer classification
- Measurable or non-measurable disease present
- No previous treatment for the primary tumor or treatments affecting study outcomes
- No hormone or chemotherapy treatments except for non-cancer related hormone therapies
- Not pregnant or nursing; women of childbearing potential must have a negative pregnancy test within 14 days before joining
- Age 18 years or older
- ECOG performance status of 0 to 2 or Karnofsky score 60% or higher
- Adequate blood counts and organ function including neutrophils, leukocytes, platelets, kidney and liver tests within specified limits
- No active autoimmune diseases except certain stable conditions not requiring systemic treatment
- No active systemic infection or positive for hepatitis B or C
- No history of conditions requiring immunosuppressant medications during the study
- HIV patients with undetectable viral load on treatment are eligible
- Cardiac function assessed as class 2B or better if history of heart disease
- No other cancers that could interfere with study safety or effectiveness
- Completed at least one cycle of treatment and eligible for surgery without unresolved adverse events
- No disease progression preventing surgery
- Predicted lung function with FEV1 and DLCO above 35%
- Registration to surgery step between 21 and 90 days after last neoadjuvant dose
You will not qualify if you...
- Patients considered unresectable or poor candidates for surgery
- Chest wall invasion, peritoneal spread, contralateral pleural involvement, involvement of mediastinal organs or vertebrae
- Metastases to contralateral intrathoracic or supraclavicular lymph nodes
- History of symptomatic interstitial lung disease
