What this Trial Is About
This phase II trial studies the effect of pembrolizumab alone compared to the usual approach (chemotherapy \[cisplatin and carboplatin\] plus radiation therapy) after surgery in treating patients with head and neck squamous cell carcinoma that has come back (recurrent) or patients with a second head and neck cancer that is not from metastasis (primary). Radiation therapy uses high energy radiation or protons to kill tumor cells and shrink tumors. Cisplatin is in a class of medications known as platinum-containing compounds. It works by killing, stopping or slowing the growth of cancer cells. Carboplatin is also in a class of medications known as platinum-containing compounds. It works in a way similar to the anticancer drug cisplatin, but may be better tolerated than cisplatin. Carboplatin works by killing, stopping or slowing the growth of cancer cells. Immunotherapy with monoclonal antibodies, such as pembrolizumab, may help the body's immune system attack the cancer and may interfere with the ability of tumor cells to grow and spread. Giving pembrolizumab alone after surgery may work better than the usual approach in shrinking recurrent or primary head and neck squamous cell carcinoma.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Patient must be between 18 and 79 years of age
- Patient must have locoregionally recurrent or second primary head and neck squamous cell carcinoma (oral cavity, oropharynx, larynx, hypopharynx) in a previously radiated field
- Patient must have undergone surgery with gross total tumor removal and be randomized within 8 weeks of surgery
- Patient must have high risk disease defined by positive margins and/or extra nodal extension
- Patient must have a PD-L1 Combined Positive Score (CPS) of 1 or higher from a certified laboratory
- Patient must have had prior radiation to more than 50% of the presurgical tumor volume with at least 45 Gy dose
- Prior radiation must have been completed at least 6 months before randomization
- Patient must have no distant disease based on imaging within 28 days before randomization
- Patient must not have received anti-PD-1/PD-L1 therapy for recurrent disease recently
- Patient must have an ECOG performance status of 0 or 1
- Patient must understand and agree to sign informed consent
- Patient must not be pregnant or breastfeeding and must use effective contraception during and 120 days after treatment
- Patient must have adequate blood counts and organ function as defined by lab tests within 28 days prior to randomization
- Patient must not have an active infection requiring treatment at randomization
- Patient must not have had non-infectious pneumonitis requiring steroids within 3 years
- Patient must not have a history of organ or stem cell transplant
- Patient must not be on immunosuppressive medication within 7 days before randomization, except certain low-dose or local steroids
- Patient must not have severe heart failure (NYHA class III or IV)
- Patient must not have received live vaccines within 30 days before treatment
- Patient must not have severe hypersensitivity to pembrolizumab or its ingredients
- Patient must not have active autoimmune disease requiring systemic treatment in past 2 years
- Patient must not have psychiatric or substance abuse disorders interfering with study
- HIV-positive patients on effective therapy with undetectable viral load are eligible
- Patient must not have active hepatitis B or C infection
You will not qualify if you...
- Patient younger than 18 or older than 79 years
- Patient without locoregional recurrence or second primary head and neck squamous cell carcinoma
- Patient who has not undergone surgery with gross total resection or randomized after 8 weeks from surgery
- Patient without high risk disease (no positive margins or extra nodal extension)
- Patient with PD-L1 Combined Positive Score (CPS) less than 1
- Patient without prior radiation to the tumor area or radiation dose less than specified
- Patient with distant disease at baseline imaging
- Patient who received recent anti-PD-1/PD-L1 therapy for recurrent disease
- Patient with ECOG performance status higher than 1
- Patient unable to consent or with impaired decision-making without authorized representative
- Pregnant or breastfeeding patients
- Patients with inadequate blood counts or organ function
- Active infection requiring systemic treatment at randomization
- History of non-infectious pneumonitis requiring steroids within 3 years
- History of solid organ or stem cell transplant
- Current use of immunosuppressive medications beyond allowed exceptions
- Severe heart failure (NYHA class III or IV)
- Recent live vaccine within 30 days prior to study drug
- Severe hypersensitivity to pembrolizumab or its components
- Active autoimmune disease needing systemic treatment in past 2 years
- Psychiatric or substance abuse disorders interfering with study participation
- Active hepatitis B or C infection
