What this Trial Is About
There is evidence for the effectiveness and feasibility of the individual components of TIER-HF-PC, however there is a need to test how these individual components interact to allow the researchers to deliver TIER-HF-PC as a model of care in its totality. In the short term, this study will assess if TIER-HF-PC is beneficial for patients and caregivers.

Timely Interventions to Enable and Reach Patients With Heart Failure, and Their Caregivers With Palliative Care
Who Can Participate
Eligibility Criteria
You may qualify if you...
- Patient is 21 years or older
- Able to communicate in English or Chinese
- Diagnosed with stage C or D heart failure as defined by ACC/AHA
- Have New York Heart Association (NYHA) functional status of 2 or higher
- Cardiologist expects survival prognosis of at least 6 months
- Experienced heart failure-related hospitalization within 6 months prior to recruitment
- Have a phone that allows telecommunication
- Caregiver is a direct, unpaid family member of the patient
- Caregiver is reported by the patient as the main person responsible for up to 4 hours of caregiving daily and/or decision maker with medical team
- Caregiver may live with or separately from the patient
- Caregiver is 21 years or older
- Caregiver is able to communicate in English or Chinese
You will not qualify if you...
- Patient has cognitive impairment such as dementia
- Patient has severe, untreated active mental illness like major depressive disorder
- Patient has a ventricular assist device implant
- Patient has non-reversible hearing or visual loss
- Patient is an active drug abuser
- Patient is already known to a palliative care service
- Caregiver has cognitive impairment such as dementia
- Caregiver has severe, untreated active mental illness like major depressive disorder
- Caregiver has non-reversible hearing or visual loss
- Caregiver is an active drug abuser
- Caregiver is a domestic helper for the patient
