Who Can Participate
Eligibility Criteria
You may qualify if you...
- Adult participants must be 18 years or older
- Self-identify as American Indian or Alaska Native
- Live within 1 hour transportation distance of Oyate Health Center in Rapid City
- Have a confirmed Type 2 Diabetes diagnosis by laboratory test
- Be a caregiver to a youth aged 10 to 25 years living in their home
- Willing to complete all study implementation and follow-up assessments
- Willing to be randomized for the intervention
- Youth support participants must be between 10 and 25 years old
- Youth must self-identify as American Indian or Alaska Native
- Youth must be willing to serve as a support person for the adult participant
- Youth must be willing to complete all study assessments and follow-ups
- Youth must be willing to be randomized for the intervention
You will not qualify if you...
- Unable to participate fully in the intervention or evaluation (e.g., planned move, residential treatment)
- Have a circumstance that may affect successful participation based on provider judgment, including pregnancy, nursing, planning pregnancy, end-stage renal disease on dialysis, or diabetes due to secondary causes like Cushing's or Cystic Fibrosis
- Any condition that may inhibit participation
- Youth participants who are cognitively or visually impaired
- Youth participants in foster care due to potential mobility issues
