Study on Locoregional Recurrence and Biomarkers in T4 Colon Cancer After Surgery

Researchers are investigating T4 locally advanced colon cancer, which has poor outcomes when locoregional recurrence occurs. This study aims to understand why stage IIBC colon cancer has worse outcomes than stage IIIA, despite similar treatments. The focus is on exploring cancer biology, metastatic mechanisms, and clinicopathology, particularly the role of peritoneal spreading as a metastatic route and the potential of serum epigenetic changes and cell-free DNA as prognostic markers. Participants will undergo radical or palliative colectomy, which aims to remove all identifiable tumors. The study will explore the role of cell-free DNA in ascites fluid as a biomarker for locoregional recurrence and investigate risk factors by comparing biomolecular markers from blood and ascites samples. This observational study follows patients after surgery to collect these data. During the study, researchers will measure and compare cell-free DNA levels and sequencing from ascites and serum over one year. Participants clinical information and biomolecular data will be correlated to identify factors related to recurrence. The studys participation duration depends on follow-up, and no investigational drugs are given. The study is sponsored by National Taiwan University Hospital and aims to improve understanding of T4 colon cancer recurrence risks.
