Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 years or older
- Acute, spontaneous, primary intracerebral hemorrhage confirmed by pre-randomization head CT
- Intracerebral hemorrhage volume 20 mL or more as measured by the ABC/2 method
- Randomization completed within 8 hours of stroke symptom onset or last known well time
- Study intervention can start within 4 hours after randomization
- Modified Rankin Score of 0 or 1 before stroke
- Informed consent obtained from patient or legal representative
You will not qualify if you...
- Infratentorial hemorrhage including midbrain, pontine, or cerebellar areas
- Ruptured aneurysm, arteriovenous malformation, vascular anomaly, Moyamoya disease, venous sinus thrombosis, mass or tumor, hemorrhagic conversion of ischemic infarct, tumor stroke, or recent intracerebral hemorrhage recurrence within 1 year
- Presence of spot sign on CT angiography
- Uncontrolled high blood pressure with systolic pressure over 220 mmHg before randomization
- Irreversible impaired brain stem function (bilateral fixed dilated pupils and extensor motor posturing), Glasgow Coma Scale 4 or less
- Hemorrhage extending to midbrain with third nerve palsy or non-reactive dilated pupils
- Intraventricular hemorrhage extending over 50% of either lateral ventricle
- Irreversible coagulopathy or known bleeding disorder
- Platelet count below 750,000 or INR over 1.4 after correction
- Need for long-term anticoagulation starting less than 30 days from current hemorrhage
- Use of two or more antithrombotic drugs before symptom onset
- Presence of mechanical heart valve
- Positive pregnancy test in females without surgical sterilization or menopause
- Allergy to urokinase
- Serious concurrent illness affecting outcome assessments including liver, kidney, digestive, lung, heart, endocrine, immune, or blood diseases
- Unable or unwilling to provide informed consent
- Life expectancy less than 6 months
- Participation in another interventional medical trial
