Eligibility Criteria

Eligible

You may qualify if you...

  • Age over 60 years
  • Diagnosed with dementia of any type
  • Cognitive Functional Scale (CFS) score greater than 1
  • Nursing home stay longer than 90 days
Not Eligible

You will not qualify if you...

  • Under 60 years of age
  • Nursing home stay less than 90 days
  • No diagnosis of dementia
  • Cognitive Functional Scale (CFS) score of 1 or less
