Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 years or older or legal age to give consent
- Baseline NIHSS score of 10 points or higher before randomization
- No significant pre-stroke disability (modified Rankin scale 0 to 2)
- Suspected acute ischemic stroke with arrival at stroke center less than 6 hours from symptom onset or wake-up stroke within 12 hours last known well
- Informed consent obtained from patient or legal representative
- Angiography suite and endovascular treatment team immediately available
You will not qualify if you...
- Known bleeding disorders, coagulation factor deficiency, or oral anticoagulant therapy with INR over 3.0
- Baseline platelet count below 30,000/µL
- Baseline blood glucose below 50 mg/dL (2.78 mmol/L)
- Severe sustained hypertension in thrombolysis patients (SBP over 185 mm Hg or DBP over 110 mm Hg), unless controlled
- Transfer center patients with recent CT/MR (less than 90 minutes old) not requiring repeat imaging
- Coma or unresponsive state (NIHSS consciousness item over 1)
- Extreme vomiting or agitation
- Seizures at stroke onset preventing baseline NIHSS
- Serious or terminal illness with life expectancy under one year
- Stroke acquired during hospital stay
- History of life-threatening allergy to contrast medium
- Cerebral vasculitis
- Pre-existing neurological or psychiatric disease affecting evaluations
- Unlikely to be available for 90-day follow-up
- Unstable clinical status requiring emergent life support
- Any condition posing hazards or affecting study participation
- Participation in conflicting drug or device trials
- Pregnant women of childbearing potential
- Exclusion per national law (e.g., age, pregnancy, breastfeeding)
- Philips employees or their cohabiting family members
