Comparing Aromatase Inhibitor Letrozole Before Misoprostol Versus Misoprostol Alone for Medical Miscarriage Treatment

This research aims to evaluate the use of an aromatase inhibitor called letrozole before misoprostol for medical termination of missed miscarriage, a common pregnancy complication where the fetus has died but pregnancy tissue remains in the uterus. The study compares the effects of letrozole plus misoprostol with misoprostol alone to find an effective and safe medical abortion method. Missed abortion occurs in 15% to 20% of clinically diagnosed pregnancies and medical methods are preferred over surgical ones due to lower cost and fewer complications. Participants will either receive letrozole tablets 2.5 mg three times daily for 3 days followed by two doses of vaginal misoprostol 400 micrograms total, given as two 200-microgram tablets 4 hours apart on day 4 or two doses of vaginal misoprostol alone same dosing schedule. This randomized, open-label study will involve about 35 women in each treatment group to compare outcomes. During the study, researchers will confirm pregnancy status and gestational age by ultrasound, check hemoglobin levels, and monitor participants for miscarriage within 10 days of treatment. The main outcome measured is successful miscarriage achievement within this period. Safety and side effects will be observed. Overall participation duration includes treatment and follow-up visits to evaluate the medical termination process.
