All Genders

Eligibility Criteria

Eligible

You may qualify if you...

  • Age 18 or above
  • Received a kidney transplant within the past year with functioning kidney (eGFR > 30 mL/min)
  • Have Type 2 Diabetes and use insulin
  • Have access to a home Wi-Fi connection
Not Eligible

You will not qualify if you...

  • Have Type 1 Diabetes
  • Taking hydroxyurea
  • Unable to wear the Dexcom G6 device at all times
  • Unable to test blood glucose 4 times a day during blinded CGM use
  • Have significant visual or cognitive impairment
  • Are illiterate
  • Are prisoners
  • Are pregnant, planning pregnancy, or breastfeeding
  • Have clinically unstable cardiovascular disease
  • Are receiving active treatment for cancer
