What this Trial Is About
VERIFY will validate biomarkers of upper extremity (UE) motor outcome in the acute ischemic stroke window for immediate use in clinical trials, and explore these biomarkers in acute intracerebral hemorrhage. VERIFY will create the first multicenter, large-scale, prospective dataset of clinical, transmagnetic stimulation (TMS), and MRI measures in the acute stroke time window.

Who Can Participate
Eligibility Criteria
You may qualify if you...
- Age 18 years or older
- Unilateral symptomatic ischemic stroke (bilateral allowed if one side is asymptomatic)
- Motor deficits in the affected upper extremity with a Shoulder Abduction and Finger Extension (SAFE) score of 8 or less within 48 to 96 hours of stroke onset
- Signed informed consent within 24 to 96 hours of stroke onset
- Willingness to comply with all study procedures and attend the Day 90 in-person visit
- Fluency in English or Spanish
You will not qualify if you...
- Upper extremity injury or condition on the affected side limiting use before the stroke
- Legal blindness
- Severe sensory loss on the affected side
- Unable to move shoulder or extend fingers on the unaffected side on command
- Isolated cerebellar stroke
- Stroke with symptoms within 30 days before the current stroke
- Participation in another stroke intervention trial after baseline assessments
- Unable to follow study procedures through 90 days
- Cognitive or communication problems preventing informed consent
- Major medical, neurological, or psychiatric conditions affecting function
- Non-cerebrovascular diagnosis with unlikely survival at 90 days
- Pregnancy
- Contraindications to noncontrast MRI (e.g., certain implants, severe claustrophobia)
- Contraindications to TMS including implanted electronic devices above the seventh cervical vertebra, ferromagnetic intracranial implants, skull defects related to current stroke, recent seizures, or previous serious adverse reaction to TMS
- Unable to perform assessments or receive TMS/MRI within required time windows after stroke onset
